Types of multiple-entry visa: for 3 months, for 6 months, for 12 months

We need following information:

1. Family name, First name and Middle name (if it is pointed out in the Passport) -
Latin spelling (Russian spelling is also advisable)
2. Date of Birth
3. Passport Number, Date of Issue, Expiry Date
4. Citizenship: as you were burn and at present
5. City and Country of Residence
6. Employment Details: Company Name, Position, Address, Telephone Number -
Latin spelling (Russian spelling is also advisable)
7. Russian Embassy you will be applying to: City and Country
8. Assumed Date of entry to Russia
9. Assumed place of accommodation: Address and Telephone Number
10. Copy of the passport data pages (the ones with photograph and signature on)

Copy of the passport should be scanned and sent us via e-mail.

All the other information could be sent us via e-mail or fax.
